Who Can Receive Care
Any student enrolled at Lincoln High School can schedule an appointment to see our medical and mental health providers by having a parent or guardian complete the registration forms and return them to the school-based health center. Copies of the registration forms are available in the health center and online (under related links on this page).
Under state law, youth may independently access some health care confidentially without parental consent, including reproductive health and mental health services.
Visit the school nurse webpage for additional health resources and information.
Family Health Insurance
Support from the Families and Education Levy does not cover the entire cost of the Teen Health Center. If you have health insurance, we ask that you complete that section of the registration form so that we can bill your insurance company.
If you do not have health insurance, please indicate that you have no insurance under the insurance section of the registration form. Our Eligibility Specialist will then help your family find coverage. For additional information about insurance, please call the Teen Health Center. No patient will be denied services due to inability to pay.
