Lincoln Athletics Registration

Welcome to Lincoln High School athletic online registration. We encourage all students to explore and participate in our athletic program.

Lincoln embraces a student-athlete model and we want all students interested in participating to have an opportunity to tryout for athletics. We work closely with our coaches and community to support students’ success and holistic development, both on and off the field.

Registration for next year is now open, please use the link for registering through FinalForms!

Registration for the 2022-2023 school year via FinalForms opens May 9. We are switching to the FinalForms electronic registration system.

FinalForms is an online Athletic forms used for Lincoln High School Athletic Registration. FinalForms allows you to safely and securely complete and sign athletic participation forms from your computer, tablet or mobile device.

FinalForms saves data from season-to-season and year-to-year, making the registration process quick and easy. Athletes may register for multiple seasons at the same time. Additionally, users may login any time to update information including emergency medical contacts and information. Users will be required to sign forms once per year and anytime an update is made.

  • Student and parent/guardian should complete this registration together because there are sections that require signatures from both. Athletes and families will complete all paperwork, upload your physical, and pay ASB through the online FinalForms portal.
  • As part of registration, please make sure you read and review the Student Athletic Handbook. All athletes and participating families are responsible for the guidelines outlined in the handbook.
  • *Make sure you have an updated Preparticipation Physical Form (within the last two years) completed by a physician and ready to be uploaded to your FinalForms profile.
    • Athletes and their families are the only ones allowed to upload physicals into FinalForms due to strict privacy laws. We do not accept or keep any athletic paperwork on file at the school. All registration is electronic.

All athletes must submit paperwork, complete the online registration and be cleared by the Athletic Director prior to participation.

Athletes registering after the season begins are not guaranteed an opportunity to tryout or a roster spot, since coaches will be determining rosters and levels of competition for each program during the first week of each season.

Due to equipment availability and for safety reasons, some sports may limit the number of athletes.

You will need the following information to allow for accurate completion of your online registration:

  • Basic Medical History & Health Information
  • Insurance Company & Policy Number
  • Doctor & Dentist Contact Information
  • Hospital Preference

Creating an Account

Parents are required to create a FinalForms account.

If you need assistance creating your account, please contact FinalForms technical support at support@finalforms.com.

Registering a Student

  1. Use the link above to create an account.
  2. Visit the FinalForms website and log in
  3. Click INCOMPLETE FORMS.
  4. Select all the sports for which you would like to register. The selection(s) can be changed at any time prior to the registration deadline.
  5. Complete each form and sign your full name in the Parent Signature field at the bottom of the page. After signing a form, click SUBMIT FORM and move on to the next form.
  6. You will see  a “Forms Finished” message when all forms are complete.
  7. If required, an email will automatically be sent to your student, prompting them to complete the forms requiring their signature.
  8. Click MY STUDENTS when you are done. Repeat steps 3-6 for additional students.
  9. If you need to update your information, click the UPDATE FORMS button.

How do I upload a document?

  1. Go to the FinalForms website
  2. Log in, and go to My Students page.
  3. Click the Folder in your student’s row.
  4. Use the popup box to upload a document.

Medical insurance is required to participate in high school athletics.

If you do not have medical insurance, we provide information regarding student accident and health insurance coverage administered by Myers-Stevens & Toohey, Inc. Coverage is available for:

  • Interscholastic Tackle Football
  • School-Time Accident Plan
  • Full-Time 24/7 Accident Plan
  • Dental Accident Plan

These low cost plans are used by many school districts across the state. Although they are not the only provider of this type of coverage, they have been recommended by our Washington Schools Risk Management Pool.

Up-to-date Physical Forms must be on file FinalForms for participation clearance. Physicals are current for two calendar years.

Don’t know if the physical is current? Log in to The Source if you were enrolled in a sport at Lincoln during the previous school year.

A current copy MUST be uploaded into an athlete’s FinalForms account for clearance. Emailed copies or paper copies of physicals are not accepted.

Students participating in Lincoln Athletics are required to pay a participation fee (ASB). These funds directly support the cost of athletic programs: game transportation, equipment, uniforms, and more.

Athletes must provide proof (receipt) for final athletic clearance. To securely purchase ASB for the 2022-23 school year you will use your SchoolPay account starting in August.

Students Not Enrolled Full-time at Lincoln

Students are required to complete a WIAA contract if they do not attend classes on campus at Lincoln full-time.

Running Start

If you are enrolled in Running Start:

  1. Complete the WIAA Running Start Contract
  2. Download a copy of your Running Start class schedule.
  3. Upload the Running Start class schedule and WIAA running start contract to your FinalForms account.

Off-Campus Classes/Alternative Schools

If you are enrolled in any off-campus classes:

  1. Confirm your neighborhood school by using the Address Look Up Tool
  2. Purchase ASB for Lincoln on SchoolPay.
  3. Upload the SchoolPay receipt to your FinalForms account.
  4. Complete the Alternative Education School Contract on the WIAA website.
  5. Download a copy of your class schedule.
  6. Upload the class schedule and the Alternative Education School Contract to your FinalForms account.

Submit Grades

Grade checks are run throughout each sports season. Please upload a copy of previous semester and most recent quarter grades to your Final Forms account.

Failure to provide grades may make you ineligible for competition. If you attend any other school for ANY classes, you must provide the Athletic Department with your grades from that school.

Off-Campus Classes/Private Schools & Home School

If you are enrolled in any off-campus classes:

  1. Complete the Alternative Education School Contract on the WIAA website.
  2. Download a copy of your class schedule for your school.
  3. Upload the class schedule and Alternative Education School Contract to your FinalForms account.

Home School

If you are home schooled:

  1. Confirm your neighborhood school by using the Address Look Up Tool
  2. Guardian/student completes SPS Online Enrollment 
  3. Once enrolled, please contact the Athletic Assistant at 206-252-2279. (Please do not email.)
  4. Upload current physical forms to your FinalForms account.
  5. Go to the SchoolPay website and purchase ASB for Lincoln.
  6. Upload the SchoolPay receipt to your FinalForms account.
  7. Complete the Home Based Education School Contract on the WIAA website.
  8. Download a copy of your class schedule for your school.
  9. Upload the class schedule and Home Based Education School Contract to your FinalForms account.

Submit Grades

Grade checks are run throughout each sports season. Please upload a copy of the previous semester and most recent quarter grades.

Failure to provide grades may make you ineligible for competition. If you attend any other school for ANY classes, you must provide the Athletic Office with your grades from that school.

Students who transfer to Lincoln must complete the WIAA Eligibility Packet

  1. Under Step 1, download the New Student Packet by selecting a language from the drop-down menu.
  2. Bring the completed form along with a current transcript to the Athletic Director. The form required for all students transferring to Lincoln who want to participate in a sport.

Note: All transfer student athletes from Private High Schools to Lincoln and student athletes seeking a 5th year of athletic eligibility are required to have an eligibility hearing with a SeaKing District 2 hearing committee before being cleared to participate in an athletic contest. All SPS academic eligibility rules apply to transferring students.
